Although I don't share your extreme level of fear, I can empathize with the basis for some of them. The world is not in very good shape, that's for sure, so some of your fears have a rational basis. But the extremity of your fears appears to be close to paranoia (layperson speaking here), including your fear of being institutionalized. If you can't trust your doctor enough to be completely honest with him/her, you're hindering your chances of getting the best treatment. Perhaps you should try to trust your doctor more. If you can't, find one you can trust. I've told my shrink EVERYTHING, including my suicidal intentions. She has SUGGESTED hospitalization but never insisted or "threatened". Yes, the world is a scary place but not as scary as a self-imposed prison.
