Adrafinil and Liver Damage -- SLS

Scott,
I have ordered Adrafinil, and the posts on adrafinil and liver damage were a little worrisome to me. What was your final take on the situation?

I take Effexor/Wellbu/Klonopin. All blood tests I've had (like for cholesteral but they check other things too) have been ok for liver. Think another one before starting Adraf. would be a good idea? My last blood test has been at least a year ago.

Safety

Although adrafinil is generally regarded as quite safe, a few adverse effects have been reported with long-term use, including stomach pain, skin irritations, and a feeling of "inner tension." Long-term use has also been associated with occasional instances of elevated liver enzymes, specifically SGOT, SGPT, GGPT, and hepatic alkaline phosphatase. Thus, if you plan to use adrafinil for an extended period of time, we suggest you have your liver function tested to establish a baseline, and then after 3 months and again every 6 months thereafter. If abnormalities appear, reducing the dose or stopping the drug should permit a return to normal.

I was going to call the drug company directly, but I had trouble with the numbers as they were listed by them. I didn't have the opportunity to retry the numbers using the France country-code: 33. It is likely that you will find someone who speaks English.

Interestingly, most of the enzymes listed above are NOT included in "standard" liver tests I've had. Indeed, different labs seem to test a different number of enzymes, although the only enzyme listed above that I've seen is alkaline phosphate.

My pdoc (who I'm beginning to wonder about...) say that only research labs can uncover most cases of liver damage caused by individual psychotropics or psychotropic combinations.

Thank you for the information.

I was wondering if you had formulated an opinion or an educated guess about the liver safety question. If so, I would be interested in hearing about your take on it.

Bottom line, I guess, is would you take it? Or let your brother/sister/mom/significant other take it and be comfortable?

I'm interested in your opinion, based on what you have heard.

Hmmm. Then if testing is unlikely to reveal liver damage, I guess I just go on faith.

I use Effexor, and that is "more likely" to cause seizures than some other ADs. And, someone once told me Klonopin turns your liver into a little black ball.

Oh, well.
It's still better with it than without it.

For anyone concerned with liver complications from whatever medication, a good idea might be to keep the dose as low as possible, but augment it with something else that is beneficial to your symptoms and rejuvenating/protecting to your liver at the same time.

What could possibly do all that? SAM-e. I'm now taking it with 300mg Adrafinil and it's working quite well. The SAM-e is not only doing wonders for my depression, it is also providing liver protection. And because the Adrafinil dose is so low, liver risk is probably a non-issue anyway.

Thanks for your response. Looks like I may have to break down and get SAMe. I was shocked at the cost when I looked for it a while back.

I have not encountered any instances on the Web in which a severe hepatic (liver) event has been described for adrafinil. There is a surprising lack of information on adrafinil (Olmifon), given its popularity overseas. I even did a search on AOL's French search engine and came up only with references dealing with its illegal use in athletic competition and its possible use in treating female incontinence.

If I had not experienced a negative reaction to Provigil, I would probably have tried adrafinil as my next therapeutic choice. It was my intention to call the drug company for some definitive information regarding side effects. Perhaps you could have the pharmacy you intend to do business with mail or FAX you a copy of the package insert.

I would encourage anyone currently receiving adrafinil to ask the pharmacy or distributor for a copy of the company package insert.

Would I try it? Yes.
Would I want a family member to try it without more information? No.

I doubt this has helped much.

Yes, your response did help. I have already ordered the adrafinil, and I will go over the package insert with my pdoc.

I also did some searches (but not in French!) and found very little. Most of what I found was pretty generic information.
