The moment you described your symptoms, I immediately considered kidney problems. There have been no human studies to consider, but rats develop serious kidney problems on lithium orotate, which are worse than those from lithium carbonate.
I suggest you see a doctor for routine kidney function tests, like BUN and creatine clearance.
It's up to you if you want to disclose the lithium orotate exposure up front, but I'd disclose it for sure upon abnormal findings. If you check out okay, I'd go on a treatment holiday period.
