Hi Jay

True, but not always by any means. Unnecessarily high doses of haloperidol are sometimes prescribed. In the treatment of the acute phase of schizophrenia, doses in excess of 7.5mg/day are rarely appropriate. Much lower doses can be effective. Doses within the range 3-7.5mg/day are well established as being effective. Doses in excess of 7.5mg/day are unlikely to be any more effective. Recent (smaller) studies suggest that less than 3mg/day haloperidol can also be effective with a lower incidence of side effects. An initial dose of 0.5mg three times a day may be appropriate in the treatment of schizophrenia.

TD is a very major concern with typical neuroleptics. Hyperprolactinemia is another frequent adverse effect. In general, none of the typical neuroleptics are an appropriate first line treatment for schizophrenia.
