This best type of magnesium is "magnesium taurate." I only know of two manufactures of it; one is "douglas labs," the other is "cardiovascular research." Of the types you listed, magnesium oxide is the only one you want to avoid since it is virtually useless. As for medication, the first thing I would try is Buspar (buspirone.) It is an anti-anxiety agent that is often prescribed to augment SSRI's in patients experiencing bruxism (teeth-grinding.) This medication cleared up a similar problem I was having and has absolutely no addiction/dependency potential so it's an easy med to get. Let us know if you decide to try it out.
