MAOIs and self education

Hi Guys,
Haven't been around much lately but thought I should mention this again. When you are taking MAOIs you have to take your level of self education up a level. A very limited number of medical professionals have any knowledge at all of this class of medication.

I went to the initial care clinic today with what they diagnosed as a viral infection (cold). The doctor knew I was taking Nardil but still suggested I take a cough syrup with dextromethorphan. I explained to him that I couldn't take that with Nardil. He asked why not, and I told him that it could possibly kill me. I don't think he really believed me, but hopefully he went and looked it up! Probably not.

Thanks Jedi, you are so right.

While I've been going through horrible nardil withdrawal this week, I made it very clear to my husband that if I became inconherent and had to go to the hospital that is was extraordinarily important for him to tell the people there that I am on an MAOI, and not only that, to stress that giving me certain drugs could literally kill me. I told him it's completely possible that even if you tell them I'm on an MAOI they may have absolutely no clue what that implies. I really need to get one of those medic alert bracelets, but it scares me that even if I wear one that say I'm on an MAOI, I could still end up being given the wrong drug.

This is very true and it scares me, if I ever were to be in the hospital, unconscious, etc. Do they know how to treat a hypertensive crisis?

I had to explain to my dentist that MAOIs were dangerous with epinephrine. He at first didn't take it seriously and kept forgetting and wanting to give me novacaine. I don't know if he looked it up, but he has been very careful since then. Unfortunately, I've had to see a lot of him because I had two crowns, including one that fell out.

I where a medic alert bracelet for parnate. The biggest danger in an ER is demerol.

Yes, if you wear the medic alert, which is is a very good idea; it should say "NO MEPERIDINE", along with the "TAKING MAOI". If you are unconscious in an accident or in the ER, this may just save your life. I put mine in my cell phone and on my driver's license, but I know the medic alert bracelet is a much better idea.

It seems to me the tyramine content of our foodstuffs is getting lower. As long as you are fairly careful, the risk of a hypertensive reaction is minimal. However, serotonin syndrome is potentially deadly and mixing the serotonin drugs with MAOIs is asking for the long dirt nap! All phenylpiperidine opioids (meperidine, methadone, tramadol) can react with the irreversible MAOIs. I've used phenelzine with many opioids that do not affect serotonin, no problem at all. Codeine, and many other opioids, that do not affect serotonin are fine with MAOIs. There is a lot of misinformation and you have to wade through the chafe!

In Australia, people being treated with MAOI's are usualy given a card by the pharmacist stating thatt hey are on drug XXX, listing the foods and drugs they should avoid, and giving advice to emergency doctors as to what they can and cant be given.

But the medic alert bracelet sounds like an excelent idea, I'd encourage anybody taking an MAOI to get one.

Methadone is a Diphenylheptane derivative, not a phenylpiperidine derivative like Meperidine and Fentanyl

Hence methadone is a safe drug to take with parnate. I know someone who is on methadone and is on parnate.

"CONTRAINDICATED: The coadministration of narcotic analgesics and monoamine oxidase inhibitors (MAOIs) may rarely result in hypotension, respiratory depression, or coma. The mechanism is unknown but may be related to additive CNS and respiratory depressant effects. Rare case reports of severe hypotensive reactions have included propoxyphene (with multiple medications), morphine, and methadone, although others have reported the safe use of MAOIs with narcotic analgesics including methadone and morphine"
