Which drug in particular? And what contraindication? There are many many psych drugs, each with its own contraindications.

I have liver cirrhosis, kidney damage and hiatal hernia so when i take medications it makes these problems worse and the medications don't even absorb and they don't even work. Should I consider ECT? I have bipolar depression and psychotic illness

Has your doctor suggested it? Many people with kidney and liver disease are able to take certain psych meds provided that the dose is appropriately adjusted and they are carefully monitored for side effects. With some drugs, renal function tests can be used to guide the dose.

Nutritional strategies? You could study up on these with web searches. You need to ask your doctor. I am not familiar with the liver metabolism requirements for vitamins.
Candidates:
5htp or tryptophan.
Taurine and magnesium.
High dose vitamin C.
Medium-high dose of all B vitamins, folate.
Medium-high dose fish oil.
Milk thistle herb for the liver.
Combination of all of these.
No matter what, the B vitamins, vitamin C, and magnesium should be the foundation for everything else you do to work properly. These particular ingredients of a cocktail take into consideration all of your symptoms.Psychiatric drugs may still play a role. You just need to take low doses of ones that are generally safe on the liver and get monitored by the doctor on a regular basis. And always combine them with milk thistle herb.

Before you do ECT (harsh, potentially damaging), I think you should give talk therapy a go. I know, it sounds less effective than meds but...even in schizophrenia and other psychotic disorders, talk therapy can be incredibly helpful. I think some sort of long-term psychotherapy could be useful, as would other non-drug interventions like group therapy, vocational rehab, writing/art, that kind of thing. Plus, non-drug therapies can be a lot more fun than drugs or ECT.
