Personally I think this doctor has to be reported immediately to the professional board he's accredited by.
And regarding your second post, I'm pretty certain it's illegal to not have a nurse present in the room during such a visit.
As far as the insurance issue goes: you're the person with the power here. The doc MUST supply you with the codes he uses for billing the insurance. I would demand a copy of all your records on file there (he'll probably charge you a buck a page, but do it anyway). Then I'd send that to your insurance company as backup for your complaint.
