Sorry, but it's either try to contact the State Department ,the Passport people in Miami ( a PAY number like a 1-900 deal!)good luck on that one! Or begin again. Getting birth certificate copies has gotten easier if you can prove you are "you". I had to get a passport last year. Went to the Post Office and got THEIR pictures (@ $10). It was sent back to me a month later because they said the pictures were too dark! Went to an Eckerd's, $8 for passport pics, three weeks later......passport!!! Got to use it last month when I went to Bolivia.
