Tryptophan (Optimax, Pacitron) uses an entirely different approach which works by supplementing the amount of serotonin rather than controlling its destruction. Tryptophan is a precursor of serotonin which is able to cross the boundary between blood and brain. Once in the brain, it is converted naturally to serotonin and so overcomes the shortage. It is usually given with a MAOI which slows down the breakdown of serotonin and enhances the effect. In a number of instances tryptophan has been responsible for a serious
blood disorder and it has now been withdrawn except for experimental purposes with named patients who have to be registered, along with their hospital specialist, with the Optimax Information and Clinical Support Unit. The treatment is confined to those with severe and disabling depressive illness of more than 2 years duration which does not respond to standard treatment and only when the tryptophan is used in combination with other antidepressants..
