I don't think its "poop out." I mean, I don't know you, and clearly I don't have an MD, but...
...you can only go so far with tranquilizers. At a certain point, some of them can actually *cause* anxiety (and not just akathisia). Something about the D2 blockade leading to the sort of anxiety commonly seen in early Parkinson's. I read a little paper on it.
I think you'd do well on 1 AAP, and then maybe use other drugs to pick up the slack. Like, I'm "Bipolar I w/Psychotic Features," which basically = "schizoaffective, from a good family." So...
...maybe something like my med combo would be good for you? One AAP, one or two mood stabilizers (I'd try just one...my doc is insistent on keeping me low dose Lamictal w/ the trileptal), plus an AD/mood-elevator, and then something PRN for anxiety, sleep?
Of course, you're different from me. Maybe...Seroquel+ a clean anti-convulsants (trileptal, lamictal) + a mood-improving, anxiety-reducing AD (I dunno why, but I was thinking Cymbalta...dosing is easier than effexor), + an anti-anxiety drug (?)
I'm not a doc, but those are common cocktails for bipolar I and schizoaffective disorders...a tranquilizer, a mood drug, an AD, maybe something for anxiety+agitation. Fairly standard treatment...
