Emsam seems to help with my chronic pain / fibro too. I'm not sure about the dose changing for the situation like you mentioned. Seems like when my situation got really bad was when I noticed how much it was helping. Keep in mind, it can take months to get the full effect. This is a characteristic of MAOI's. The longer you take it the better you will feel. Trileptal doesn't seem to interact with many meds. I wouldn't worry too much.
