It seems that standard antidepressants do not suit you well. I know it's old school but have you tried dothiepin? Some docs don't prescribe it because it's dangerous in overdose, but your doctor gave you amitriptyline - so this can't be too much of a concern! There's also doxepin, but although it's a potent antihistamine, I'm unsure how well it works as an AD.

I wonder whether any of the so-called mood stabilisers would reduce your agitation/restless eg. carbamazepine.

Shame that psychiatry is basically trial and error....
