A proportion of amitriptyline is converted to nortriptyline in the liver. Amitriptyline is much more likely to cause sedation than nortriptyline. Amitriptyline is best started at a very low dose eg. 10mg, or even 5mg given your response to nortriptyline. You can then increase the dose as tolerated.
