"therapeutic nihilism" is a fancy pants term for the approach some (I'd say a lot) of shrinks have to mental illness, especially severe mental illness. It basically means they'll throw you drugs, give you ECT, throw you in a hospital, etc., but they don't expect a whole lot of real progress.
