Is that really 20mg or 200mg? I have a family member who was on 200mg twice per day. Later switched to Nuvigil (it is more pure and lasts longer - you take it just once per day).
I know the person sometimes was allowed to double up on a morning dose of the Provigil. Had no side-effects.
Other people are quite sensitive and cannot tolerate even 25mg. It can cause anxiety in some people. Some get headaches. Some worse. And some - nothing.
