Possibly not. I would simply add the Topamax to Lamictal. Sometimes Lamictal + lithium works to treat rapid cyclicity or a recurrent course. There is no reason not to add Topamax *slowly* to the other drugs you are taking. Just drink plenty of water to help prevent the production of kidney stones.

If multiple mood stabilizers proves inadequate, then I would look to add an antipsychotic like Geodon or Abilify. Zyprexa would almost certainly work better, but it is known to dysregulate glucose and lipid dynamics, and can produce diabetes or Syndrome-X.
