If you absolutely cannot find a doctor to Rx an MAOI, order it from xxx or a chemical supply house and self-medicate. Then, when you are [hopefully] feeling better, you will be in a state of mind to convince a pdoc to Rx one for you.
If I had to bet on which MAOI would be most likely to work, I'd go with 1mg/kg phenelzine hands down.
the response to high-dose tranylcypromine is often robust, but it is prohibitively expensive mail-order (12-20 tabs/day!)
