You can use any opiate/opioid as long as it does not have significant serotonergic activity. The big no-no opioid is demerol/meperidine/pethidine; under no circumstances should that be used concomitantly with an MAOI. Ultram is sketchy; not as bad as demerol, but it is a demerol analogue nonetheless and I would avoid it.

DXM, while devoid of opioid agonist activity, I should mention also has serotonin releasing action and thus cannot be taken with an MAOI.

I personally have used heroin and buprenorphine while on tranylcypromine, and buprenorphine on phenelzine with no ill effects whatsoever. High-dose tranylcypromine is especially ideal for those on opioids because it mitigates the sedation they can cause, if that is desired.
