The thing to keep in mind is that sublingual uptake is going to bypass the first-pass metabolism. All the substances absorbed across the gut wall enter the portal circulation, and pass through the liver before moving on to the body. Liver enzymes readily degrade selegiline, as do enzymes in the gut wall. In effect, sublingual doses are more potent because more parent selegiline is available to the brain. But that brings up a possibly contentious issue.
Sublingual uptake is so rapid and thorough that there is a sudden spike in blood concentration. This spike, if large enough, will also disable MAO-A to some extent. Depending on your genetic makeup, your total MAO and the ratio of MAO-A to MAO-B can vary substantially. Some people might be very sensitive to tyramine, even though the total dose of selegiline is low. You can't assume that you will not have a tyramine reaction, even at 5 or 10 mg.
The patch releases selegiline so slowly that the peak plasma concentration does not spike. That makes it more likely that MAO-A will survive unscathed, and remain protective against tyramine.
