Guanfacine + NAC for PTSD, TBI, ADHD, long Covid

Twelve patients with "brain fog" including difficulties in executive functions were treated with guanfacine (1mg, PO [by mouth] bedtime for the first month, increased to 2mg after 1 month, if well-tolerated) and 600 mg NAC daily.

Guanfacine+NAC improved cognitive abilities in eight of the twelve patients; four patients discontinued therapy, two for unspecified reasons and two due to hypotension and/or dizziness, common side effects of guanfacine.

Those who stayed on guanfacine+NAC reported improved working memory, concentration, and executive functions, including a resumption of normal workloads. One patient briefly stopped taking guanfacine due to a hypotensive episode and reported a return of cognitive deficits that abated with resumed guanfacine treatment.

Does guanfacine produce depression as a side effect as often as clonidine does?

I don't think guanfacine is as strong a sedative as clonidine. I honestly did not find clonidine depressing, but just that it slows you down. I think because of the norepinephrine suppression, it also makes you achy and tired. But that is dose dependent.

Which is better for ADHD?

Clonidine is notorious for producing a very real depressive state. A doctor gave my maternal grandmother clonidine for hypertension. She had no history of having a depressive mood disorder. Within a week of beginning clonidine, she became visibly depressed. She laid on the couch and watched Judge Judy all day long. She didn't pick up a book. Reading was her greatest joy. Fortunately, I was well aware of depression being a sequala of taking clonidine. Once she discontinued clonidine, her depression lifted within a few days.

Guanfacine without question. The evidence was strong enough that Shire was able to get their special form of guanfacine, Intuniv, FDA-approved for the treatment of ADHD.
