If cannot get MAOI, what med combo to mimic MAOI?

Posted by Chinaboy on March 25, 2015, at 8:36:11

If you absolutely cannot get MAOI, what med combo will you use to make it mimic MAOI?

I know that Nardil and Parnate are like magic pills to many, but they are not easily available.

Why not just make a combo out of the easily available Meds to make it mimic the magic power of MAOI?

What's your opinion?

If the efficacy of an MAOI is what you require, I suggest finding a doctor who can appreciate your treatment needs.

You can attempt to put together a combination of medications that will target a diverse profile of neurotransmitters, but it will not be the same as taking an MAOI.

That being said, olanzapine (Zyprexa) in combination with antidepressants has been shown to have promise. The combination of olanzapine + fluoxetine (Prozac) = Symbyax has the strongest evidence in bipolar depression. The combination is also approved for treatment-resistant unipolar depression. I personally take olanzapine with Marplan, along with desipramine and others, to good effect.

Now finding a doctor that combines MAOIs with TCAs, that's a trip. Add in a stimulant (I take Nuvigil) and it's damn near impossible. But a doctor that will accept MAOIs in general is not that difficult to come by if you pre-screen and ask before meeting them. I use the psychiatrist section of the Psychology Today website and send out mass emails.

Why would you absolutely be unable to get an MAOI? Are they illegal in your country? I don't understand.
If you are in the US, you can certainly get an MAOI. Older psychiatrists are usually more experienced and comfortable prescribing them. If you can't find a doctor familiar with MAOIs and willing to try them, see if you can get an appointment with a major psychiatric center - often a good teaching hospital. They will likely have experience with MAOIs.

There is no way of mimicking the action of MAOIs with other drugs, I'm afraid. Although other meds act on some of the same neurotransmitters, they affect them in a different way.

I think I recall reading that, in one small study, Effexor+Remeron (California Rocketfuel) was just as effective as Parnate, with fewer side effects.

That study used a maximum dose of Parnate of 30mg. Hardly a fair comparison.

And many sources claim MAOIs have severe "side effects" because of the dietary and drug interactions, but these are not side-effects.

what about some kinda standard AD plus Ritalin or an amphetamine? That's not exactly standard treatment, but I think its more common that it used to be...

You can't really mimic an MAOI.
