If the efficacy of an MAOI is what you require, I suggest finding a doctor who can appreciate your treatment needs.
You can attempt to put together a combination of medications that will target a diverse profile of neurotransmitters, but it will not be the same as taking an MAOI.
That being said, olanzapine (Zyprexa) in combination with antidepressants has been shown to have promise. The combination of olanzapine + fluoxetine (Prozac) = Symbyax has the strongest evidence in bipolar depression. The combination is also approved for treatment-resistant unipolar depression. I personally take olanzapine with Marplan, along with desipramine and others, to good effect.
Now finding a doctor that combines MAOIs with TCAs, that's a trip. Add in a stimulant (I take Nuvigil) and it's damn near impossible. But a doctor that will accept MAOIs in general is not that difficult to come by if you pre-screen and ask before meeting them. I use the psychiatrist section of the Psychology Today website and send out mass emails.
