Zyprexa/Olanzapine

Posted by Bob on October 17, 2010, at 16:48:44

Is Zyprexa still considered among common if not first-line treatments, either alone or as an augmentation strategy, or is it a last resort these days because of the metabolic and weight gain dangers?

My former pdoc said she never Rx's zyprexa anymore; apparently, even if you don't gain weight on it, you can still get diabetes. I guess she was scared for her patients (and scared of lawsuits, of course).

I've read online that Geodon and Abilify are becoming more popular as first-line agents. Apparently the QTC interval deal with Geodon isn't so bad, after all.

And what does your current pdoc have to say?

Zyprexa is a suitable first-line treatment for schizophrenia and severe acute manic episodes. It's not first-line for anything else.

I guess what I really mean then, is whether Zyprexa would be a last resort augmenter to other antidepressants over other atypical APs? Or would some pdocs still reach for it in this regard before another AAP?

I don't really know the answer to your question. In my opinion, it should be first line. There are risks with all meds. With zyprexa, it requires a change of diet that sees sugar as an enemy. Which it already was anyway without zyprexa. So that's actually a good thing.

Second in line would probably be abilify. After that it is a toss up with the rest.

The above opinions are from simply following pbabble post by post for a number of years. Just going on real world results in this particular population.

Unfortunately it seems quite a few lawsuits for zyprexa on TV now. Diabetes and weight gain not caused by food from the medication. I've read it's great for suicidal thoughts. Phillipa

No, it could be chosen before other atypicals in some cases. For example, it might be suitable if there was a lot of agitation.

Zyprexa is one of the highest selling drugs out there. I think its the highest selling AP. I take it and don't have diabetes, been taking it for 12 years or more. I got diabettes from taking high doses of geodon. Since being back on zyprexa the diabettes went away. I personaly think zyprexa is the best AP out there. No I don't work for zyprexa. I sure hope it becomes generic in Apr.!

oh, by the way I'm losing weight on 15mg of zyprexa now. Does that mean I'm getting placibo's now, I don't know. Seems like its working though. The pills are a year old does that mean anything?

I found all the APs -- and I tired all but clozaril -- stopped suicidal thoughts and ended vegetative depression in its tracks. But I gained weight on all of them, even abilify. 2-3 pounds a week. I didn't eat more and I actually upped my exercise, since I was not depressed. But the weight just piled on. It didn't come off when I stopped APs either. I'm lucky I didn't get diabetes.

It's great to hear that Zyprexa has worked for you. Did you avoid weight problems on it also?

So I assume you found something better to treat whatever the APs were supposed to be addressing?

No, you won't be.

If it's been stored appropriately in the original packaging, a year is not very old at all.

It has turned my life around! No, I have not avoided weight gain. When I started zyprexa I was about 150lb, 5'10". Now I'm 205lb. I've lost weight before on Z and gained. I made it up to 220lb and down to 170lb while taking it. I've been losing weight again now.

Whats odd is I can tell its working, so it must be real.

I'm just trying to understand why I would need a higher dose all of a sudden. Must have been that stint on abilify for 6 months.
