MAOIers Poll: Did u suggest a MAOI or pdoc?

We MAOIers here know quite a bit--many of us (myself included) tried every SSRI/SNRI, remeron, Wellbutrin....

Funny thing I suffered so long and I was the one who brought up MAOIs--I'm not bitter, but really my shrink shouldn't have had to wait for me to bring it up...honestly. without the internet, I would might have ended a fate much worse than my solvable problems (Axis IV stuff, hehe.)

And I wonder if, I hadn't done my own research, if I would have gone through ECT, which was recommended to ECT.. It's a shame there's such a bad rap about hypertensive crisis....

I hope, somewhere in a lab, someone is playing around with nonselective inhibitors of MAO--perhaps with some novel properties, or reversibility.

Thoughts, anyone?

Hi,

I am considering making the leap to MAOIs --Parnate specifically--and I am scared to death of both the food and drug interactions. I worry that I might eat chocolate without thinking, take a cold medicine by accident, etc. I'm thinking I have to wear a medical ID bracelet too.

Shoot when EMSAM was new my pdoc gave me samples said maybe a piece only of the patch. Then she decided it was too activating for me. I really liked the idea of a patch and if back side effectd could remove and could see that no more med on or in body. Phillipa

I take parnate and the food interactions are not that big a problem Chocolate is fine, unless you eat ounces and ounces of it. I drink coffee. No problem. I have small amounts of parmesan and feta - no problem. The drug interactions also aren't that huge a deal. I take lamictal -- no interaction. And Perphenazine -- no interaction. You have to avoid SSRIs, SNRIs and atypical antipyschotics. But, with the exception of atypicals which made me gain tons of weight, I never benefited from SNRIs or SSRIs anyway.

I wish my p-doc had put me on parnate sooner. He tried ensam first, which did nothing for me. I saw a consultant at Harvard Medical School who felt ensam was overrated and wanted me to try parnate and perphenazine. Old drugs, but they worked wonders for me.

My pdocs suggested MAOIs. They are at a university clinic, so they might be "by the book" and "by the book" MAOIs are a fourth class of effective meds people with TRDs should certainly try. I take Nardil and it has helped a lot. The dietary restrictions are NOT THAT BAD. The University of Pittsburgh Medical Center has the best list and it is available online somewhere. Some list of dietary restrictions are old and list just about everything imaginable.

The UPMC diet is outdated, too, as it still says no on mozzarella cheese. Mozzarella isn't aged and is thus fine to consume in whatever amounts (gotta love my cheese sticks!!). Shulman and Walker 1999 found that double pepperoni/double mozzarella pizzas from major chains contained no tyramine-- I'm still careful on the pepperoni though because I'm paranoid but I think other sources have said fine on the mozzarella too. It just goes to show that most of the diets are overly restrictive.

Ah, yes, I eat fresh mozzarella (basil/olive oil/tomato) in MASS quantities. That's right, it's not aged. I'm disappointed with UPMC now! But I like how they categorize things less oppressively into (paraphrasing) "NO" "TAKE IT EASY" and "CHOW DOWN". It's better than the typical YES/NO duality.

Thank you so much for the information. Um, does this mean I have to wean off Seroquel while I wean off Prozac? This could be a nightmare.

I did the washout in a hospital, but at that time I was severely depressed and suicidal. Personally, I never had much in the way of withdrawal effects from any meds. Certainly not the antipsychotics. The only side effect they gave me was weight gain and I was completely unaware of them in my body.

Hi,
I have taken Parnate and Seroquel together in the past. They were no problem except I was tired all of the time because of my reaction to histamine blockers. I have take micro doses of Seroquel(12.5 to 25mg) for MAOI induced insomnia.

The Medscape Drug Interaction Calculator(http://www.medscape.com/druginfo/druginterchecker) found no interactions between Parnate and Seroquel.

PS The first time I ever used a MAOI it was suggested by a psychiatrist. This was 12 years ago. Ever since it has always been my idea. One PDOC absolutely refused to put me on a MAOI, even though this is the only class of medications that has ever worked for me. I fought this guy for months before giving up and finding a GP who would prescribe for me.

I went into the pdoc office with the intent of trying Nardil! (social phobia/depression) He, of course, made me try a snri and ssri first with no effect before obliging to Nardil.
The hypertensive crisis is waaay overexaggerated. I can eat/drink lots of stuff on the "banned list". That list is just a cautionary safe-all that doesnt apply to most people.

Ha! Lucky (relatively speaking) the man or woman who has a pdoc with the testicular fortitude to suggest an MAOI. It takes extra amounts of courage and knowledge for a doctor these days to prescribe an MAOI at all, let alone suggest it.
