Antipsychotic + Stimulant . Prevent tolerance?

Posted by maz81 on August 9, 2008, at 7:34:11

My pdoc has put me on 400 mg of Seroquel (quetiapine) and is planning to add Adderall to help with bipolar depression and ADHD. His logic for using an antipsychotic is to prevent hypomania.

I am wondering whether the Seroquel will block the Adderall from working as it would seem to me that they are working against each other. Although I only have a very basic understanding of psychopharmacology.

However if the combination does work, I was wondering whether the combo could prevent stimulant tolerance. I understand that dopamine antagonist such as antipsychotics increase the number of dopamine receptors and make them more sensitive to dopamine. Or maybe the two drugs just cancel each other out?

Does anyone have any experience or insights?

I think it is impossible to tell from a theory standpoint what will happen with any combination. I know that when I was on Zyprexa, and then added Ritalin, I could sure as heck feel the Ritalin. Zyprexa might have muffled it a bit, but it was still strong.

The only thing I know of that has some theory to it as well as real-life anecdotal evidence to prevent tolerance of stimulants is the addition of Memantine.

I don't know if the AP will prevent stimulant tolerance, but an amphetamine+ some sort of tranquilizer (barbiturate, antipsychotic, meprobamate) is a pretty old-school way to handle mixed anxiety/depression and bipolar disorders. Back in the day there were a lot of these combo pills, like Steladex (stelazine plus dexedrine), ThoraDex (thorazine plus dexedrine) and Dexamyl (dexamyl with amobarbital, a barbiturate-type sedative).
Apparently, some people responded quite well to these sorts of combos...I would imagine the adderall might help mitigate seroquel-induced lethary and weight gain.

Still, be careful and look out for side effects, such as...movement disorders, raised blood sugar, psychosis, agitation, so on and so forth.

Good luck!
