Anybody combining SSRIs/SNRIs??

Hey folks:

I am just curious...if there are any folks out there who may be combining an SSRI (Proz, Zoloft,Celexa, etc) with one of the SNRI's (Effexor, Serzone, etc.) I have been reading about some of the combos of SNRIs and SSRIs with a fair amount of success (mind you, n=very small!!)

I am really encouraged by the folks who try and be as creative as possible.

William, i tried taking serzone along with Effexor-XR, but finally stopped the serzone because it didn't seem to do anything extra for my depression or ocd. Also, i have read that people have combined prozac with snri's. Apparently (ask Cam for sure) as long as you watch the dosages, you aren't in too much danger of serotonin syndrome.

1) Effexor is *not* an SNRI. The only SNRI I know of is Reboxetine. Effexor is a SRI because it affects seratonin-mediated synapses at doses up to 150-225mg.s and "recruits NE-mediated synapses above that dose level. Combining Effexor with SSRIs is a pretty good prescription for Seratonin Syndrome.

2) Check out Dr. Bob's Tips section for info on Serzone + SSRIs. (Type Serzone in the search line, then select "Nefadozone + SSRIs" from the search results.)

3) The wisest babblers here research prospective med changes carefully, check it out with other babblers, and then take the results to their pdoc!They also keep in mind babbler watch-phrases such as YMMV, and start low, go slow.

SSRI+SNRI combos are being used successfully. In Dr Bob's Tips there is mention of a patient who combined Prozac+Effexor due to insurance restrictions. The end result was excellent results with no side effects. Right here at this forum, a regular (Racer) was doing terribly with Effexor. But shortly after adding Prozac to it, she recovered dramatically and quickly. There have been other cases too. A very similar strategy--combining SSRI with tricyclics--has been used successfully for decades. My GPs favorite strategy is to add a norepinephrine tricyclic to an SSRI. He also combines Serzone with Zoloft or Celexa fairly often. But it doesn't mix well with Prozac. SSRIs+SNRIs are not uncommon or new.

Right now I am very interested in switching to an SNRI (reboxetine, mirtazapine, nefazadone) possibly in combo with an SSRI. Effexor is not officially classed as an SSRI but actually is one but it also acts on dopamine receptors. My psych calls it a super-SSRI. I took it for three years with the same side effects and results that people get with Prozac, Paxil, and Zoloft. I am currently taking Paxil.
I want to try reboxetine but it is not yet available in US. I am willing to try mirtazapine but am worried about whether I will feel sedated during the day. My biggest problem with the SSRI's is terrible sleep at night, major fatigue during the day, and am losing cognitive function.
Reboxetine sounds promising for me at the right dose and if the side effects are managable.
