TCA suggestions

Posted by rculater on February 25, 2012, at 9:54:09

I have tried Ami which had too many cognitive effects and then Nort which made me on edge, angry, irritable.

I suffer from Atypical depression but don't want to go down the MAOI route (yet) besides, getting a doc to prescribe MAOI in the UK is a challenge.

SSRIs had the normal dulling, lethargic , fattening, sexual effects (apart from prozac but that was short lived) and Effexor was Ok but i dont ever want to go through the withdrawal of that again.

I heard a lot of good thinks about Imipramine ?

Suggestions ? there seems to be a lot of choice of TCAs

Hope you are all well

I took Tofranil-PM plus other stuff for severe psychotic depression once. It helped, although I did end up getting a bit "angry." That could have been the Rx amphetamine, though.

Anyway, Tofranil is good for severe, endogenous depression with agitation or psychomotor retardation, or so the experts say.

Could you try endronax plus remeron or something? I know, 2 drugs, kinda complicated, but just based on my limited experience it might be easier on you to take 2 more modern ADs than to take 1 old school TCA. Just throwing that out there for you.

I used to take desipramine. I don't think it worked any better than an SSRI. What really helped was Abilify. Have you tried an atypical?

Best,

What dosage did you arrive at? Any bipolar stuff going on? How was your energy level while taking nortriptyline? Was it better than without nortriptyline.

You could try desipramine. It, too, can be activating and produce irritability, but these things sometimes dissipate as one emerges from depression. For me, I respond better to nortriptyline than desipramine. However, there are people for whom the converse is true.

I was on low doses of both got up to 20mg.
Energy was OK on Nort but not on Ami. Im going to try imipramine. I also have migraine variant im trying to treat (vertigo/dizzyness) but that could be related to anxiety/depression.

It's an effective med. Common side effects are dry mouth, dizziness when standing up, sweating and increased heart rate. It sometimes causes drowsiness at first but far less than amitriptyline. Don't stand up too quickly because you might feel faint. The dose needs to be increased gradually.

Hope it helps!

Do you think clomipramine should be a first-line TCA treatment? It was one of the least tolerable drugs that I have tried.

You could try a serotonin receptor antagonist.
nefazodone, trazodone and all the atypical APs do that.
Also I think doxycycline, an antibiotic, does something with these receptors but it may not be a beneficial effect.

I would definitely be one of the first TCAs I'd prescribe for OCD. It once held the distinction of being the gold standard in treating depression, so maybe it should be considered first line.

How did clomipramine affect you?

Clomipramine reduced my general anxiety and helped to decrease intrusive and recurrent thoughts. I couldn't live with the side effects, however (anorgasmia), so I switched to Viibryd.
