I have been on Nardil for about 8.5 weeks, and have had 4 weeks of anorgasmia - not delayed, I just can't have one no matter how much sex I have, how much I masturbate. i just don't get that sexual feeling. I can get erections just fine, but just can't relieve myself.... any help?

If you could get a script for Periactin (cyproheptidine) p.r.n it might help. It works by blocking 5-HT2 receptors, which Nardil may be overstimulating. Somehow 5-HT2 receptors seem to be involved in sexual response. I discovered a little bit of Zyprexa can help too, probably via the same mechanism. I had the same problem on Paxil and other SSRIs so I know how you feel - like having an itch just out of reach.

Is Anorgasmia one of the Nardil side-effects that goes away over time, or are we stuck with this for good???

Sorry, that wasn't aimed at anyone in particular. I just keep seeing the same subjects/complaints posted here over & over & if people just took the time to look through the archives a bit, often on the same page(!), or use the search box below then answers will come quick and fast.
Questions like these have been answered so many time on here that it gets kinda repetative.

Its still working very well. At least when I have sex that is. Haven't been in the mood lately due to stress. But I continue to take 1g per day, usually in 2 doses of 500mg each on empty stomache.
Sex can still take a while, especially if I'm not really turned on to start with (which I dont think is usually a problem for men being the opportunists that we are lol) but who likes to rush? ;) Masturbation usually takes 5 minutes so I know its working well. As a side note, I had COMPLETE anorgasmia before I started HGW. And yes I'm on an MAOI (Marplan 50mg).
I plan to finnish the tub off (which lasts for 3 months at 1g day) then see if the problem has fixed itself, which it normally does with MAOIs. Just a matter of time :)

Horney goat weed is just the same as a plant extract of a viagra like substance - I took the real thing it didn't help with orgasm at all!!

Trouble is with periactin is that it says on the packet very clearly DO NOT USE WITH MAO-Is - so i am steering clear - did you actually use it with an mao-i?

Hi squatlover (by the way, what are squats?)
I am currently taking 1g of Horny Goat Weed (also known as Epimedium) per day, standardised at 20% icariin - THE active ingredient in HGW. AND YES, I am taking an MAOI with it - Marplan at 50mg/day. It is working VERY well at counteracting the anorgasmia. But it is NOT contained in any formula. It is pure HGW herbal extract made by and sold by a particular company. It contains NO other herbs and is a very potent product especially at the sugested dosage (1g/day). The problem with many hebal products is that they contain variable amounts, qualities & standardisations from company to company, product to product that you cannot simply rule out its benifits just because one particular product didn't work. St.Johns Wort is a CLASSIC example. I have tried many different brands of St.Johns Wort & none have worked except one, and that was Kira LI-160. And DAMN is that a good antidepressant! Just a pitty it didn't help the anxiety :(
So just because the brand you went for did diddly s**t for you, doesn't mean that its an ineffective product that wont help you - herbal products VERY RARELY work for me & this is one of the very few that has. And like I said before, even the majority of brands of a particular herb will prove useless - Ginseng is another example. I've only ever found one usefull Ginseng product, & that was from a chinese herbal shop & came in a jar with a consistancy like thick treacle or mollasses. Far unlike all the powdered capsule & pill forms you get at the health store. If you do have one more blast at HGW then at least try one which someone has actually had success from themselves, which is usually a good sign that its a half decent product deserving some credit.The fact about periactin (cyproheptadine) is that people HAVE used it with substantial success to counteract anorgasmia caused by MAOIs, Nardil in particular. It has also been used together with Bethanechol for anorgasmia, again caused by MAOIs. The current problem with MAOIs is that the large majority of docs & pdocs simply aren't familiar enough with them to feel comfortable trying various other meds out with them. Yet MANY people have & are using so'called contra-indicated meds with them - amphetamines, SSRIs (yes HONESTLY!), Tryptophan (Optimax) & 5-HTP, Wellbutrin, Provigil, etc, etc, etc......
I myself am prescribed 300mg Wellbutrin with my Marplan & also when I was on Nardil too. 450mg on Nardil infact. I also currently take 50mg 5-HTP 3-4 times per week wich is a great enhancement, yet NOTHING resembling the slightest bit of Serotonin Syndrome & I've been doing that since early Jan.At the end of the day, if your not happy about trying something out then there is only one answer for it - seek proffesional medical advice! :)

No, I haven't taken Periactin with an MAOI - it's only available on prescription where I live, but I would consider taking it with Nardil. My guess is that the manufacturers included a generic 'Do Not Use With MAOI'-type warning just to cover their backs. There are no serious (i.e. life-threatening/hypertensive crisis etc.) interactions reported between Periactin and Nardil though. The most I could find was a report of visual hallucinations and a decrease in the antidepressant effect of Nardil when combined with cyproheptadine:

Minor Drug-Drug Interaction cyproheptadine and phenelzine (Minor Drug-Drug)
Cyproheptadine may antagonize the therapeutic effect of monoamine oxidase inhibitors (MAOIs). This interaction was reported in one patient treated with phenelzine for chronic depression. Another patient developed visual hallucinations following the addition of cyproheptadine to a stabilized regimen of phenelzine. Causality has not been definitely established. The mechanism is unknown but may be related to the antiserotonergic activity of cyproheptadine. Patients should be monitored for potentially diminished therapeutic response to MAOIs during coadministration with cyproheptadine. Discontinuation of cyproheptadine may be necessary if an interaction is suspected.
__________________________________________________I remember wanting to take diphenhydramine (Benadryl) with Parnate years ago, but was scared off by exactly the same type of warning in the package insert. Turns out many people here have been advised to try Benadryl for MAOI insomnia by their doctors. I've herad quite a few using it with no problems. I understand you being wary though - I wouldn't expect you to risk a major drug interaction just based on something you read on a message board, but I thik it does bear looking into a little further considering how annoying this side-effect is. Could you run it by your doctor?
