I do it all the time. I don't see it as a psychotic symptom. Sometimes, if I'm upset, it's dissociative. Other times it's meds or just one of those things that happens when I get tired or am not at my best. I also do word substitution at those times. My family is used to mentally (or verbally) putting in the correct word, but it can be confusing to others. And sometimes it's because I really wasn't paying much attention to what I was saying.

Are there some times when it happens more than others?
