Oh, you sound like me. I have to be VERY careful about potential nicknames, and even I can't foresee what I come up with.
My Daddy used to do it with me, and I do it with my pets and thankfully to a lesser extent with my son - where I confine it to odd endearments along the "my little cabbage" variety.
The one that most offended was with a very distinguished and elegant male dog who I insisted on calling something along the lines of "Mahly" or "Mawly". My husband would swear he'd roll his eyes at me and telepathically send a highly insulted "Mawly?!!" each time.
The most technically awful was "Geldyburger".
