If your family knows you are sick, then just say "I'm sorry, but I have to leave, I'm not feeling well" Your family will understand, and they'll let you go. Hopefully, no embarrassing questions.
Even if they don't know, you should be able to do the same. Its not unusual for people to leave when they really need to.
Go and spend time with your siblings, and enjoy it, but don't stay so long that you suffer because of it.
