I have some experience from previous works.

Questions to ask:
Is the paranoia worst prior to, during or after hallucinations???

"psychological ramifications" does he remember his hallucinations and able to articulate them after or during
ie, during, is he aware that he is actively hallucinating.

Delusional Disorder of the paranoid nature--I don't know your hubby, but is his age a factor the doc has ruled out eg, often this can happen with age related dementia.

Abig help is noting the Frequency, Durations and the intensity of each episode or on a scale if it is continual and give to his pdoc...
