Matt,I understand that there are limits on how far you can distance yourself from the parental units when you're young, but can't you find a way to go to doctor's visits on your own? I don't know how a doctor can treat you effectively with you and your mother fighting in his office.
My advice, based on limited information, is to set up an individual appointment with your doctor asap, and in the meantime spend as little time as possible around your mother. Spend as much time out of the house as you can.
