I'm not a therapist, but in my experience pain and fear can cause this kind of reaction. A person in pain is not always "rational" and a comment questioning their behavior may be misinterpreted as not believing that they hurt. A person who is afraid may act "badly" due to the fear and then be embarrassed or ashamed and act even worse as a defense.

The whole thing sounds really awful. And sadly familiar.
