Effexor Levels- SE: IR vs XR

I'm curious about this because my doc and I are trying to find the right way to deal with my effexor situation.
Over the past month, it's been intersting with effexor... We started with IR (had activation problems with IR, even at only 37.5 MGS a day) tried XR at 37.5, then wanted IR again for more mileage at a higher dose (I wasn;t dosing evenly on the IR and that was one original hypothesis as to why it wasn activvating), and now we're back to XR at 37.5 ... so I've been on a ride.

The highest I've been on was the 50 MGS of IR daily (25 MGS 2 X day). 50 MGS of the IR makes me feel extremely activated... I couldn't sleep, had vivid dreams, severe restless regs, etc... I took that dose for about a week or so.
So Now, I'm back at 37.5 of the XR... Lately I've felt anxiety ridden, uncomfortable, etc.. I take Valium and we're adding Topamax back in.
On the flip side... I can actually sleep now more than 3 hours a night on the XR, though I still have vivid dreams. So here are my questions....

1) Are these effects the result of my body getting used to the XR?
OR
2) Are these withdrawal effects from the drop from 50 MGS of IR (25 MGS twice a day) to 37.5 XR once a day?
OR
3) A combination of the two from switching back and forth between them?

Finally, how long should I wait these feelings/ side effects out before going back to the doc to say "back to the drawing board"?
