I don't think about it too much personally. I've been on medication so long that I'm used to the idea that it just keeps me well, rather than cures anything.
It seems like paroxetine is essential for you, so you may as well optimise the dose. If 50mg or 60mg is better than 40mg, it could be worthwhile even though you'd rather not be on it.
