I didn't know Lithium caused sexual side effects, everything I've read said it did nothing, or improved the stuff that happens in the bedroom.
Marplan however is very similar to Nardil, and it's known to cause constipation, postural hypotension, erectile dysfunction, insomnia, sedation and so on. These side effects do normally increase with higher dose.
The positive is that a lot of the side effects tend to lessen or disappear after being a couple of months on the medication.
Trying to lower or remove Nortriptyline or Lithium sounds like a good idea, if it doesn't give satisfying results you could just reinstate the original dose.
