IME, the less a doctor is impressed by newer trends in psychiatry (DSM, the new drugs and stuff) and therefore the less optimistic they are, the more likely they are to Rx MAOIs.
I have never had any trouble getting doctors to consider it, but I have a sense of whom to ask.
It may not be just age related.
