I would just suggest that perhaps adding the energizing TCA drug desipramine to an SSRI might offer some benefit. This strategy might be no better than the others, but it is something often used by some psychiatrists to produce a more robust antidepressant effect. However, the Wellbutrin + Zoloft (Welloft) combination is also popular. If the fatigue doesn't dissipate, you can try switching to Effexor.
