My version of Nick's story:
When I go to see my doctor there are typically four or five people waiting for a doctor. In five minutes, or thirty-five minutes he walks into the waiting room and nods at me (he doesn't use names, to preserve confidentiality. I see him for five minutes, maybe ten if there is a lot to cover, he writes my prescriptions then I pay his assistant $180. After a few months my insurance company pays $60 of this.
