I had a doctor like that. Because he could barely remember me or my history, I would type out a brief summary and my recommendations and give it to the receptionist upon check-in. After going to his office, sometimes he would not notice the note, and I would remind him, it's there, in your clipboard. Then he would read it and the conversation would start in my direction.
