Hmm,
Depression with bad anxiety sounds a lot like me. I finally figured out it was the anxiety that was the main component, and the depression was subsequent to it.
Like you, I failed all AD trials...the SSRI's even made me worse.
Once I addressed the anxiety situation, the depression nearly vanished.
Now I'm taking Klonopin and Nardil. Around 1.5 - 2.0 mg of Klonopin abates the panic attacks that cause my depressed state.
The Nardil is a newer addition. I'm seeing some interesting effects from it...far different from any SSRI. It's probably too early to judge it, but it seems quite promising.
Anyway, I digress. Have you tried getting really aggressive with managing the anxiety, to see if the depression is secondary to it?
CBT was also extremely helpful for anxiety (especially panic and the associated anticipatory anxiety about panic attacks). CBT is always worth a try, in my opinion. Some here will say otherwise, but I think it's always worth a shot.
Anyway, in the opinion of many here, depression can be secondary to anxiety, and sometimes anxiety is the "etiologic factor". Perhaps you should try getting really aggressive with treating that, and you may find your depression simply vanishing along with it.
Also, a guy who posts here named Viridis has had tremendous success combining ADD medications (Adderall, I believe) with Klonopin. His symptoms sound similar to yours. His posts are very insightful, perhaps you could ask him.
