I have also felt very scared. When my brother's behavior was unstable (unmotivated laughs while walking around, loud music the whole night, giving away my parents belongings, silently following you very close to your back, throwing glass next to you if he considers that you had traspassed "his area" in the dinning room, physical resistance when we call for medical asistance to give him medicine through a shot, etc...)I just thought that that period will be over. To really bad periods, good ones follow (we talk, we go out, we laugh, he helps me with advice). He never takes med. when he is at home. He just takes it in the hospital.
It is great that your wife is having some agency in her recovery by trying that med. I have read on this list that some people on a new med. are waiting for three weeks to see results. If it doesn't work both of you won't lose more time and look for an alternative. In both situations you win something... but of course I hope that it works. Searching through this site I found a manual for self recovery. It states that while people are in the stable period they could write what procedures they want to be taken when they "loose it". IN that way, even though they are unstable, they have some agency in their lifes.
