Why is ECT so hit and Miss?

I don't get it, istn't there some kind of predictor for who will respond well to ECT and who won't.

I've read such conflicting reports, some people saying it wrecked their life and others who say that it saved theirs. Why is it such a crap shoot?

Surely they must do MRI/PECT scans before and after the treament to see if there is some kind of correlation between those who respond and those who don't

It seems such a shame that there is no way of telling.
