I don't have OCD, but I've heard of opioids being used for it. Ultram may take a few days to work because its active metabolite, desmethyltramadol, needs time to accumulate.

I only found one case report:
Goldsmith TB, Shapira NA, Keck PE Jr. Rapid remission of OCD with tramadol hydrochloride. Am J Psychiatry. 1999 Apr;156(4):660-1.

but I'm pretty certain I've seen some reference to morphine treatment of OCD, too. I'll try and see if I can dig it up.
