Yikes Sal, it sounds like tough one.

You have likely tried these, but just in case...

Remeron? Remeron will knock you out fast, like within half an hour or an hour from dosing. Small amounts like 7.5mg work much better than higher doses. Of all the things I've tried (which is everything), 7.5mg Remeron was by far the most sedating. I even found tiny amounts such as 2.5mg or 5mg to be intensely sedating. At 15mg it lost the sedation and instead turned into a more activating type drug. Remeron on super low doses could do the trick. I was always amazed how such a little piece of crumb from the pill could be so powerful, and how the smaller the dose the higher the sedation. Weird.

Elavil, or other very sedating tricyclic? I don't like the side effects, but you seemed to do fine with Clomipramine, so maybe you can tolerate the side effects better than me.

Tiny dose Remeron is what I would try though, if you haven't already. Small doses are totally unlike regular doses.
