I used to take Prozac for OCD (I'm no longer taking anything for it - at least not for now). I started at 20 mg and noticed some improvement after about a month to six weeks. I then went to 40 mg and noticed more improvement (again, it took about another month to notice it).

I would say that at 40 mg, my symptoms were at least 80% gone. It was almost as if I didn't have OCD any longer -- but then I do have a mild case. It is annoying and causes me a lot of inconvenience on a daily basis, but it isn't disabling.

I also have a cousin with OCD who was (probably still is, I just haven't asked recently) having good success at 60 mg and I have heard that 80 mg is not uncommon.

I'm not sure if the severity of symptoms has a direct correlation with the dosage required for relief. I do know that even among people with similar degrees of afflictedness, the response to different doses and time needed to see the response can vary.
