Hi George. That's a tough one. Finding the right doc is sometimes harder than treating the depression. Both at the same time is, well, I've been there, quite frustrating. Hang in there. In the meantime I do have some suggestions for a plan of action.

First, we need to get you feeling better so you'll have the strength and creativity to go about prospecting for a doc. I've said this many times on the net and in person, and I'll say it again because I feel strongly about it. Take the money you would have spent on a doc visit and go to your local Wal*Mart and get a couple boxes of Nature Made SAMe ($20/box). Start with one pill mid-morning on empty stomach, one more in mid-afternoon empty stomach. Double the dose on day two. Then up it to 6 pills (3 twice a day) on the third day. Stay there and see how it goes. SAMe may or may not be a cure, but it can sure lift you out of the hole considerably and quickly with little or no side effects. That will allow you go on and search for a doc. I've tried this over and over and notice much improved overall relief as early as day three. You gotta try it. With or without an AD, I think you'll find it to be an effective stepping stone to get some relief NOW.

Then begin the search. Call local hospitals and clinics for references. Ask who they would recommend to treat their own family. Ask for someone known for success with difficult cases. Approach docs at random (yellow pages) by phone and/or letter briefly describing your condition and treatment history. Of those who respond to you, you may find you really like one or two. A gut feeling. That "chemistry" thing. If there is a university nearby, call them and ask around for a recommendation. You know what kind of doc you would like to have, so use that as a screening process.

Basically it involves some creativity and detective work. Knock on enough doors and one of them is bound to be the right one. But to do that, you need strength and relief, so start with the SAMe. I've been in your shoes, and may likely be there again sometime, and this is exactly what I would do in your shoes.
