good self-assessment for bipolar/depression

Does anyone know of a good self-assessment for depression/bipolar? The scales used in most of the studies are based on clinical observation. I would like to have something that I can self-administer weekly or so to help monitor where I'm at with my depressive symptoms. I am more concerned with depressive symptoms than with mania or anxiety. Let me know. Thanks!
